As a self-pay, non insured patient, you are entitled to a special rates on your services today.
By accepting care from us, you agree to pay in full for your visits as outlined below.

DMV, Sports, School & Camp Physicals. No Appointment Needed! Whether your child is getting ready for school or camp, a physical is the first step in making sure they’re healthy and ready for activities!

DOT Physicals

$75 Each

A Department of Transportation (DOT) physical exam is valid for up to 24 months. Our physical exam helps commercial drivers fulfill the Department of Transportation’s requirements.

Immigration Physical and Vaccination

$150 Each

myDoc offers immigration physical services, performed by a licensed Civil Surgeon designated by the U.S. Citizenship & Immigration Services. In addition, we carry vaccinations required.

Pre-Employment Physicals

PPD Free if Required
$75 Per Physical

Matching workers to the physical demands and essential functions of their jobs.
Performed by our myDoc licensed practitioner. Group rates are also available. *Employer Account Required.

In-House Labs

“In-House Labs” refer to lab testing performed with results in the office, for example, a rapid strep or a urinalysis.


Some medications administered during your office visit will be charged separately.  Please confirm with our staff during your visit if you are receiving injections or IV’s


Labs collected and sent out for results will be billed by the lab as a separate charge.

Outside Entity

Medicine, equipment, or supplies prescribed by our staff and filled at an outside entity are not covered in your visit costs with us today.


We accept all major insurances. If your insurance is not on our list of accepted insurance plans, this could mean that we are an “out of network provider”. You will pay us first, then submit claims through your insurance company. If you don’t have insurance coverage or from an different country, we have an affordable self-pay pricing schedule – very comparable to “in network provider pricing list”, which is typically about 1/3 of the ER costs.
